InterCommunity Health Care provides a number of ways for people experiencing mental health issues to get support, build skills, and stay healthy. Our programs vary in intensity/hours of care to meet the needs of our clients and provide them the best care possible. Services are provided face-to-face, by phone, and through Zoom.

InterCommunity Health Care provides a number of ways for people experiencing mental health issues to get support, build skills, and stay healthy. Our programs vary in intensity/hours of care to meet the needs of our clients and provide them the best care possible. Services are provided face-to-face, by phone, and through Zoom.

Care coordination services include:

Care coordination programs:

Behavioral Home Health (BHH)

Behavioral Home Health (BHH) provides care management for Medicaid recipients with severe mental illness. Clients receive one hour of services monthly from a BHH Care Coordinator. Only current InterCommunity clients are eligible to receive these services. 

Community Support Services (CSS)

Community Support Services (CSS) provide care coordination and management for people with severe and persistent mental illness to help them build skills in nutrition, hygiene, relationships, health management, money management, medication management, coping skills, and other areas. Clients receive services 1-4 times per month.

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) provides therapy, care coordination and management, and in-home medical support for people with severe and persistent mental illness. The goal of this program is to help clients understand how to take care of themselves better, and to use the proper services when they are not feeling well. Clients receive services three times per week.
