We are serving the communities of Hartford, East Hartford, South Windsor, Bloomfield, and Wethersfield.

The team consists of a clinician, care coordinator, and peer recovery specialists. We will be starting services via telehealth, but the goal eventually will be in-home services. Please call us at 860-569-5900, Ext. 321 for further details and questions. Click here for a flier.
